Store Setup > Forms and labels > Form types > Select Add button

What is the dialog used for?

• Use this dialog to create a new form type or to edit an existing one.

Fields & buttons	Description
Description	Enter a description for your form type. Descriptions must be unique. The system does not allow 2 form types with the same description.
